Chronic Obstructive Pulmonary Disease (COPD) Program - Cowichan

Provides COPD assessment, education and COPD flare-up support for people with COPD.
The Cowichan Community Health Services (CHS) COPD program provides COPD assessment, education and COPD flare-up support for people with a confirmed (or pending) diagnosis of COPD with moderate to severe symptoms.

The service aims to increase your confidence in self-managing COPD, improving your quality of life by reducing symptoms severity and decreasing the need to be hospitalized. An interdisciplinary health care team will work with clients and their caregivers to address the following: COPD education, creating an action (exacerbation) plan to deal with flare-ups, medication use, energy conservation, airway clearance, pursed lip breathing for relief of breathlessness, exercise conditioning plans, making healthy choices, motivation for smoking cessation or behaviour change and advance care planning. The program may include the following:
  • Telephone consultation and follow up
  • Clinic visits
  • In-person home visits for clients unable to attend clinic
  • Home health monitoring
  • Exercise and/or education programs


Referrals can be self-referral or physician and can be made through Community Access Services at 250-739-5749. The criteria for referral is:
  • In the past 12 months, at least one presentation to the Family Medicine clinic, Emergency department or admission to the hospital for exacerbation of COPD or pneumonia.
  • Confirmed or pending diagnosis of COPD with MRC score of 2-5
  • Able to participate in COPD program (education, support and exercise)
  • Willingness to be referred
  • Lives in the Cowichan Valley CHS catchment area

The team will be providing service across both Ladysmith and Duncan. They will be using clinic space at 121 Ingram as well as Ladysmith health centre, and will also do home visits as appropriate.

Service is available in English.
